Seeking Part-time Senior Care Provider In Lakewood

I need senior care provider with experience. Our care concerns are: little mobility assistance and transportation. We ask that you handle transportation to and from my aunt's home to her dialysis 3x's/week (just 5 mins from where she lives in Lakewood). She goes at 5 a.m. and stays until 10 a.m. when pick up is needed. After pickup, getting her situated at home until about 11:30 a.m. would be great on M/W/F. She is friendly, kind and suffering from diabetes complications and needing help getting to dialysis, occasionally to dr appts or sitting in on dr appts( primary doctor comes to her house). We would like someone who's reliable, safe, friendly and patient. References as well. Looking for help asap. Thank u!
