About who needs care: Dependent in all ADL(s), high fall risk, recently diagnosed W/vascular dementia, confused, incontinent, needs frequent reminders, is a DNR, no tube feeding, dementia has not reached maximum level (moderate), cognition poor, a prior accomplished music player (piano, pipe organ), lots of confabulation, enjoys nature; generally pleasant but of recent times no -compliant W/meds , bathing, & eating. About the care needs: Someone schooled in traditional value system( respect for the elderly), doesn't mind hearing the same story over & over; can sit & maintain safety W/her. Services needed include: companionship, feeding, bathing / dressing, and mobility assistance.
