About who needs care: I broke my back in an automobile accident in 2017, and then broke my left hip six months later. 2021, I had a stroke. After the stroke, I learned to walk and feed myself again three months. However, I now live in an independent living facility and need assistance with someone taking me to my medical appointments. About the care needs: I would like for someone to accommodate me to my appointments, and complete paperwork when needed, because I have severe glaucoma in both eyes and am blind in my left eye. Services needed include: transportation.
