In Home Care For 86 Year Old Grandmother

My family and I are looking for care for our precious Grams. She is 86 years old and the heart of our family. We are committed to keeping her in the comfort of her home. However, she needs more day to day support that we unfortunately can't provide consistently. She has light dementia and atrial fibrillation. She lives alone and for the most part takes good care of herself. She needs help with : 1. Transportation- grocery store, hair / doctor appointments 2. Household help- small chores and light cooking 3. Medication- she is having a hard time organizing and remembering her medications 4. Companionship- She is very lonely and mainly just wants someone to talk with and be with her. Grams is the world to us and we are very involved and deeply close to her. This is why the right fit matters. We hope you can be an extension of our family. We want to find the right person to help her continue living a happy life at home for as long as possible.
