Job Overview:
Uloma Immigrants' House is seeking a compassionate and skilled Home Provider with a Nursing background to offer specialized, supportive care in a home setting for individuals in need of health-focused assistance. The Home Provider will combine their healthcare expertise with a nurturing approach to provide residents with both a safe, stable environment and the healthcare attention they require. This role is crucial to ensuring our residents feel supported, respected, and empowered as they transition toward independent living.
Key Responsibilities:
Health Monitoring and Support: Conduct regular health assessments, monitor vital signs, administer medications as prescribed, and address basic health needs.
Daily Living Assistance: Support residents with daily activities such as personal care, hygiene, meal preparation, and mobility, ensuring a safe and comfortable environment.
Care Planning and Documentation: Develop and maintain individualized care plans and health records, documenting any changes in health, behavior, or daily routines.
Educational Guidance: Educate residents on personal health management, self-care practices, and preventative measures to enhance their independence.
Coordination with Healthcare Providers: Facilitate access to external medical resources, schedule appointments, and collaborate with healthcare professionals to ensure comprehensive care.
Emotional Support and Advocacy: Provide compassionate, patient-centered support, addressing emotional needs and fostering a sense of security and belonging.
Emergency Response: Respond to medical emergencies with appropriate interventions and follow-up care as required.
Qualifications:
Required: Licensed Nurse (RN or LPN) or Certified Nursing Assistant (CNA) with a minimum of 2 years of experience in healthcare or home care.
Strong knowledge of basic nursing care and first aid procedures.
Experience working with culturally diverse populations and individuals with varying health needs.
Compassionate, empathetic, and culturally aware approach to caregiving.
Ability to work independently, manage time effectively, and maintain a stable home environment.
Excellent communication skills and the ability to work as part of a multidisciplinary team.
Preferred: Bilingual or multilingual skills to enhance communication with residents of diverse backgrounds.
Must have a valid driver's license, reliable transportation, and insurance.
