About who needs care: My mother is self-sufficient for now, when at home with her boyfriend/roommate. She has mid-level dementia managed by a neurologist. We just need someone to coordinate and transport to/from doctor visits, manage prescriptions and pill-boxes. This would include summarizing doctor visit information/results for us and facilitating flow of information between doctors. About the care needs: Someone who -is able to manage scheduling and attendance of Dr. appts -is able to facilitate communication between doctors and family -is proficient in managing prescriptions and pill boxes -has patience with answering the same question multiple times -is able to provide home visits on an as-needed basis Services needed include: transportation.
