About who needs care: Patient suffers from Parkinson's Disease, which makes mobility and memory challenging. Patient also has cancer, and numerous medical appointments. Right now, patient is receiving in-home health care, but will move to external care when he is able. Patient is friendly and affable, and a former athlete and cyclist. Help is needed with activities of daily living (bathing, dressing, getting ready for the day and at end of day for bed, feeding and meal prep), as well as physical support for moving back and forth from bed to wheelchair, and to the toilet. Daily schedule: 1. Patient wakes up around 7AM, gets ready for the day around 8AM, getting out of bed, getting dressed and eating breakfast. 2. Patient then reads, engages in conversation or watches TV until lunchtime, eats lunch and takes a nap in the early-mid afternoon. 3. After the nap, the patient again spends time reading, talking or watching TV until dinnertime, eats dinner, and after dinner gets ready for bed and goes to bed around 8-9 p.m. About the care needs: Patient enjoys engaging with a friendly, warm person who can also be firm with him about needs (like taking medicine, eating, etc). Skills needed would be related to mobility (being able to safely help the patient move from bed to chair, toilet, etc, using body weight mechanics). Knowledge of elder-appropriate exercise and movement is very helpful. Willingness and ability to help with patient-related food prep, laundry and cleaning up will be needed in the times when the patient is resting/sitting quietly. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
