Care Manager 10-20 Hours/week Position requires management of all aspects of care for 65-year-old male recovering from a stroke. Patient currently has 24 hr care. He is wheelchair bound and requires assistance with all ADL's except feeding. The Care Manager would be responsible for the following: - Maintaining caregiver schedule, providing supervision and payroll - Prepping weekly medications - Refilling prescriptions - Making/attending medical appointments, providing advocacy - Scheduling transportation - Ordering supplies - Review daily care sheets to identify/resolve issues that require modifying medications or other interventions - Provide a written weekly summary Minimum position requirements include: - LPN/RN preferred - Excellent organizational skills - Supervisory experience - Excellent communication skills
