About who needs care: Parkinson's Disease. Fell Dec 9 and has comminuted fracture of left upper humerus, fracture of left femur, subarachnoid hemorrhage in brain. Hospitalized 3 weeks, 1 at Rush oak Park Park Hospital, 2 at Gottlieb rehab. He had surgery Dec 10 to repair hip fracture. He is in a fracture brace on left arm. His bladder worked going into hosp and does not work now. He has a Foley catheter. He is in a wheel chair. He cannot walk on his own. He cannot use a cane or a walker. He cannot get out of bed on his own. He cannot stand and brush his teet on his own. He cannot bathe himself. Hobbies are reading, chess, watching food channel, watching sports on TV. He has in home therapy, with PT and 0T and he has to do exercises on his own 2 to 3 times a day. He needs overnight urine collection bag changed to leg bag in AM and back to overniite bag at hedtime. Leg Bag has to be emptied several times a day. He has to be cleaned in shower a few times a week and daily sponge bath including genitals due to Foley catheter. He has several doctors appmnts every week and needs help to transfer in and out of car. He has had both upper and lower teeth pulled and he is on soft mechanical diet. . About the care needs: He is a retired professor in the biological sciences, he is educated . He likes sports and the food channel and classical and rock and roll music. He has a good sense of humor. He needs a caretaker who can help him sit on the toilet and get up ftom a toilet. He needs someone who can help him exercise and can practice walking in the hallway. He needs someone who can help with shoulder exercises. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
