Evening Care Needed For My Uncle In Chicago

Seeking a reliable caregiver for my uncle, Alan. He has mild dementia and uses a walker but is mobile. He has a main caregiver during the day so I'm looking for someone Monday-Friday from 5-9 p.m. Evening responsibilities would include dinner prep (cooking or helping to heat/plate food and getting him set up), helping change his clothes for the night, teeth brushing, getting him into bed, occasional laundry, and of course companionship. We're looking for someone to maintain consistent hours but can be flexible if need be.
