Assistance With ADLS For ALS Patient In Atlanta

Seeking care for myself in my 40s. Services needed: bathing / dressing, mobility assistance, light housekeeping, errands / shopping. I am 46 and have ALS. I live with my wife and two kids. I am in a wheelchair and need help with activities for daily life (showering, range of motion, dressing, brushing teeth, changing catheter). I would also like help with other tasks around the house such as opening boxes, putting away groceries and laundry, changing lightbulbs, .... I am looking for someone who can follow directions. My first priority is help with ADLs. However it be great if they could help out with tasks around the house as well. I am looking for help 4 hours a day, two or three days a week.
