Companion Care Needed For My Grandmother

I am in need of someone to assist me with my grandmother. She has lived with me since 2012. I am chronically ill and I have a small child of my own. We are newer to the area and pretty much as soon as we moved to the area my grandmother had a horrible fall which landed her in the hospital and the doctor's recommended that she go to a skilled nursing facility. Her health has been up and down since that time, and she has not been able to come home yet. As of right now I just need help getting her transported to and from appointments. I would need someone to attend the appointments and take notes for me. My grandma has some cognitive deficits so you would need to assist her at the appointments with paperwork and such. I need someone patient and very caring. The days of the week would depend on her appointment times.
