About who needs care: 40 yr old male goes to dialysis 3x week and is pending surgery on this knees so he can walk again and resume living independently. Has been approved of "Long Term Care" through Florida Medicaid. PCP has authorized 35-hours and or Live in Assistance. is in need of assistance for daily activities of daily living. cooking , bathing, light housekeeping. About the care needs: Need a male to assist with transfers to and from wheel chair and shower chair, some assistance with dressing to and from doctor's appointments. Services needed include: meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
