Care For Immobile 25 Year Old Once A Week

I am a 25-year-old old female who has limited mobility and needs help completing daily household chores, completing errands, and to drive me to my weekly appointment. Daily household chores include; washing, drying and folding clothes. Vacuuming and mopping. Dusting. Light cooking. Feed both of my dogs. Let my lab out into our backyard to throw the ball with him. Running grocery errands on my behalf. Our pug, Onyx, requires very little. He is blind so he spends most his time lounging on the rug. Our lab Ace is 9 months old and professionally trained. He is a extremely well behaved dog but still a puppy so he has a lot of energy. I have a doctor appointment once a week 20 min from my house. I'd like someone who is able to help me get in and out of the car and make sure I don't collapse between the car and the building.
