One-Time Transportation Needed To And From Hospital

I am having surgery on my broken ankle on Thursday 2/27 and need transportation from my home in Clearwater to Morton Plant North Bay Hospital in New Port Richey. (About 25 miles and 50 minutes, according to Google Maps). Arrival at hospital will be early a. m. (5-6) and I will be discharged sometime late afternoon (exact times to be determined after I meet with surgeon on Wednesday). I am open to anyone that can provide either both parts of the round trip or a half of the round trip, the morning arrival being the most important. I'll cover fuel cost as well.
