About who needs care: Severe heart problems- multiple heart attacks, severe stenosis of mitral valve and aortic valve and are inoperable End stage rental disease and on hemodialysis 3x a week Very weak after recent major heart attack and 11 day hospital stay About the care needs: Understanding Patient Kind Respectful Skills- able to move him around house, in and out of bed, wheelchair/car, bath him, assist in restroom Services needed include: transportation, feeding, bathing / dressing, and mobility assistance.
