R: Client is alert and oriented. Client lives with his wife M. Client uses a walker. Client has medical history of congestive heart failure, low blood pressure, kidney failure, and is oxygen dependent. Client will require standby assistance with dressing and almost full assistance with bathing. Client wears adult depends and has a urinal bottle that may need to be emptied in the morning. Client wears hearing aids. Client is a fall risk and sometimes has a hard time with swallowing food. Light housekeeping includes dishes, laundry, and taking kitchen trash out. Meal prep will be cooking and heating and serving, client is on a low sodium diet. Caregiver will drive client to dialysis appointments.
M: Client is mostly alert and oriented. Client lives with her husband R. Client sometimes uses a cane. Client has medical history of mild dementia, UTIs, hip replacements, stroke, and high blood pressure. Client is mostly self directed with ADLs, but will require reminders and encouragement to shower/bathe and brush her teeth. Client wears adult depends so may need help changing those. Client could be a little hesitant with new people due to dementia diagnosis. Client may talk a bit slowly sometimes due to stroke. Client likes to go to backyard and water plants. Caregiver could go for walks with client down the street or in the backyard. Caregiver will drive client to grocery store or mall, etc. at times.
