Hands-on Care Needed For My Husband

I need a hands-on care provider experience for my husband in his 70s. Preferences for a caregiver who is CPR / first-aid trained. He is 76, has had 3 strokes, affecting his right hand & right leg, foot. Needs daily exercise, can walk with assistance, but not too far. Needs daily wheel chair rides outside. Needs help with transferring from bed to wheel chair, from wheel chair to recliner, from wheel chair to commode which he does all 3 with walker. He is 6'1, 233 lbs. Loves to go shopping. Can get into car from wheel chair.
