About who needs care: Lives in Assisted Living facility. She has moderate stage dementia and is quite inactive. She's recovering from a fall Sept 1 that resulted in a partial hip replacement and fractured right wrist. Tends to sleep, eat, nap and watch TV. Needs motivation to get moving. About the care needs: Some experience with PT to encourage her to work on mobility would be helpful. Services needed include: mobility assistance.
