No aggressive behaviors. Lives at Eastvale 92880 near Chino with adult sons family, is often home alone during the daytime Sun-Sat but family may be present in the home Approved for 35 hrs/week through IHSS, can work 7 hour shifts starting as early as 8:30 a.m. but with flexibility to start and end later if needed Car available for provider to drive, provider must be willing to drive client Requires the following care: Ambulation: Has limited mobility (uses a walker) and falls easily, needs assistance with tasks (cannot multitask) Bathing: Can shower by himself but needs supervision occasionally (to prevent falls) Dressing: Selecting and putting on clothes - often Toileting: Has limited mobility so may need supervision nearby but not assistance Feeding: Can feed himself fine but needs help cooking, plating food and cleaning up afterward Personal hygiene: Self-care tasks like grooming, cutting nails, and shaving Transportation to and from doctors appointments (including in LA) and other errands like going to the post office, bank, grocery store, barbershop, nail salon Advocating for the client while he communicates with medical professionals, help reporting symptoms/issues Supervision while exercising (including stationary bike) to prevent falls Sometimes has bladder/constipation problems that may require medical attention Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, feeding, bathing / dressing, and mobility assistance.
