Seeking Part-time Senior Care Provider In Phoenix

I need a senior care provider with experience for my 72-year-old father who has recently been diagnosed with dementia. He is independent for his ADLs and making his own meals, goes to the gym regularly and is fairly healthy. We need someone to drive him to and from the gym or grocery store when we aren't home and to keep him company. We would like someone who can make sure he takes his medication on time and who can be patient when he is confused.
