About who needs care: Dementia patient. Wheelchair bound. Incontinence. Will need help bathing, lifting, transporting to doctor visits. Not sure what else. About the care needs: Mom I being discharged from nursing home. Her short term memory is almost non existent. She has been constantly in a wheel chair for over a year and food has to be pureed. Services needed include: transportation, feeding, bathing / dressing, and mobility assistance.
