I need transportation from my home beginning 10 a.m. on February 28, at Sand Lake, MI, to my doctor's office at Grand Rapids, MI. for a 11:00 a.m. appointment. Also, on the same date, I needed to be picked up at 1:00 p.m. on February 28, from same place to be returned home at Sand Lake MI.
