Hospice is a unique specially formulated care team —doctor, nurse, social worker, spiritual care coordinator, home health-aide and when needed personal volunteer—collaborating to treat your physical, medical, spiritual, existential, financial, and family’s needs. We treat the “whole person” with a team approach to care that can be requested or referred by a physician. Hospice is covered through the Medicare benefit for patients with a life expectancy of six months or less. We step in when you are ready to transition out of the cycle of aggressive, disease-centered treatment that can often be painful in the hospital. Every person has the right to live pain-free with dignity, at home, surrounded by family and friends. We can help make that wish a reality for you and your family. Our nurses are on call 24-7 and we transform your home into a safe, comfortable environment that meets your medical needs. Our team makes routine visits that provide a safety net of support for you and your family.
