Victoria's Place Senior Care is Geriatric care management fee-for-service company that provide consultation and care management services to older adults and their families to attain the highest quality of life. Our services include weekly/monthly home supervisory visit, creating, evaluating and updating care plan, and making appropriate recommendations. The care manager can make referrals for crisis intervention, appropriate physician, elder law attorneys, counseling and support groups. Can also coordinate/oversee home care, communicate with family members, coordinate outside services, arrange for care companions, and assist with placement as client needs change.
