Uptown Homecare

Uptown Homecare was created by Houstonians to specialize in providing private duty nurses, aides, sitters and companions for our neighbors who are recovering from an illness, have just had surgery or need long term care and support.

Our services include around the clock nursing or companion care, meal preparation, running errands and transportation to the doctor's office. We care for all diagnosis, including dementia, cancer, heart failure, diabetes, COPD, stroke and Alzheimer's disease. We provide care for patents on ventilators, are quadriplegic or paraplegic, or just need a little help to stay independent and at home. Our services are available in the home or hospital.

Care Coordination
- Coordination of all home health services
- Updates to physician and family as requested
- Communication with other health care providers
- Periodic review of needs/services
- Teaching independent living strategies
- Coordination with Home Health or Hospice Provider

Personal Care Services (HHA/CNA)
- Personal hygiene assistance
- Routine transfer assistance
- Bathing/shower assistance
- Assistance with nutrition & eating
- Assistance with exercise
- Help with bathroom use
- Skin care/nail care/shaving
- Assist with walking/mobility
