About Texas Care Management Llc

TXCM has one goal and that is to help families care for their loved ones who meet one of the following criteria:

- They are elderly or disabled and are unable to keep up with housekeeping like they used to.
- They are in a nursing home or facility and you would like them to have extra care.
- They are elderly or disabled and would benefit from someone who prepares their meals for them for the week.
- They are elderly or disabled and can't seem to keep up with medical appointments andreliable transportation to those appointments. <br/>- They are elderly or disabled and all of their family live far away and are unable to check on them to see how they are doing. <br/>- They are elderly or disabled and live on their own, they have become isolated from society and could benefit from companionship and socialization. <br/> <br/>TXCM offers these services and so much more! <br/> <br/>Step one is to schedule a consultation so we can get to know one another and figure out if we're a good match for your family! Then we create an individual service plan for the client that will serve as a road map of what is expected from the family and what TXCM will provide.
