My name is Ivonne N and I am the Director of Nursing at Team Select, a family-centered, patient-focused home care agency covering all region 8 counties (Lee, Collier, Sarasota, De Soto, Charlotte, Glades, and Hendry). Our company provides skilled nursing services for both pediatric and adult patients. Team Select is dedicated to serving and helping those in need. For this reason, it is with great pleasure that we are offering Pro Bono Skilled Nursing Visits for patients that are uninsured/underinsured within thecommunity including medication reconciliation/administration, teach and train for proper patient care, weight management/monitoring, etc. We accept Private Pay and all insurances besides Medicaid for those that are insured. Within the next few months, we will also be offering PDN (Private Duty Nurse) hourly services as needed for medically complex patients with G-tubes/J-tubes/GJ-Tubes, tracheostomy, vent dependent, etc. We are striving to partner with the surrounding PT/OT/Speech Therapy agencies to collaborate and offer total, holistic care for all of our patients in the near future. If you have any patients whom you may not be able to provide services to, please feel free to send their information our way to determine if we may be able to accommodate their needs.
