SOS Eldercare services to seniors and special needs persons, including those with Alzheimer's and Dementia:

Home care. Companionship, socialization, recreation, assistance with shopping, errands, meal preparation, and transportation arrangements.

Obtaining services and access to community resources.

Regular supportive and/or supervisory visits to the elderly in their home.

Social Worker Accompaniment to doctor 's offices. Liaison with family regarding questions, notes, and passing of information to family, power of attorney, guardian, or other designated person.

Assisting with health care choices and alternative living. Organizing an elder-friendly move to a new home or facility.

Assisting in resolution of family conflicts.

Care Management for clients with Developmental Delays, Alzheimer 's, Dementia.

Advocacy for proper care of elders in Nursing Homes and Assisted Living Facilities when families are at a distance. Liaison with physicians, psychiatrists, psychologists, elder care attorneys, nurse practitioners, dieticians, physical and recreational therapists.
