Qualicare Family Homecare provides services to all ages and all levels of care needs. Our services include companionship, assistance with activities of daily living such as meal preparation, bathing, toileting, light housekeeping, grocery shopping, errands and other outings, and more.
Qualicare staff promote positive outcomes for hospitalized clients. Our early involvement and proactive program helps facilitate discharge from the hospital, ensuring a safe, smooth transition to home. Our transitional care program allows a caregiver to be with the client discharging from hospital or moving from home to assisted living, and remain with them during the transition to their new environment or get settled in at home. We provide services from four hours up to 24/7. Get well. Get home. Stay home.
Advocacy and professional representation at visits with other healthcare professionals is provided by a registered nurse. Our RN will prepare written communication to the provider in advance of the appointment and provide the client and family with a summary of discussion afterwords. She will ensure that everyone involved is up to date and has the same understanding of the planned care.
360 Case management means we will coordinate all aspects of care and arrange for additional services needed by the family and client, to optimize the family relationship and decrease stress for everyone.
