What is PHCS? We are a home care provider with over 20 years of experience serving the Metro Milwaukee community. We are a state licensed and federally certified home care provider. The State carries out regular inspections/surveys of our home care agency to insure we are in compliance with State and Federal standards. Our Home Care team consists of seasoned home care professionals that have the knowledge and expertise to properly care for you and your loved ones.

What can PHCS do for you? We will develop aHome Health Plan of Care in conjunction with your Physician. The Plan of Care is custom tailored for the individual client based on the client and their family s needs. We will coordinate and monitor our home care services with a multidisciplinary team of health care professionals so your home care goals are met whether they are skilled services provided by a licensed health care Professional or non-medical services that will assist you to stay independent and safe in your home and our Milwaukee community. What makes PHCS different? We understand that as home care providers, we are guests in your home. All members of our Care Team are experienced, thoroughly screened, and bonded. You can rest assured that you can feel completely safe with our caregivers. We will respect your health care needs and goals, your privacy, your belongings, and your home. All of our caregivers are trustworthy, and receive ongoing training to provide an advanced standard of care. Being a State licensed skilled home care provider, we can provide the same skilled services as our larger competitors (Horizon Home Care or the Visiting Nurses Association, VNA.)
