Personal Health Care provides service to infants with complex medical needs to older adults needing assistance with Activities of Daily Living and everything in-between.

Our Staff includes Registered Nurses (RN), Licensed Practical Nurses (LPN), and Home Health Aides (HHA) whose focus is the one-on-one support of our clients and their needs. Personal Health Care has the ability to provide nursing or aide support for any of our client's needs.

Personal Health Care provides comprehensive case management for every client. Our multidisciplinary healthcare team will assess the unique medical and personal needs of the client and create a plan that ensures safety and wellness while maintaining maximal independence and privacy. We assign one case manager to handle all of the client's needs, increasing continuity, communication, and client satisfaction. Our case managers will ensure that the appropriate staff meets the needs of the client.

All employees hired by Personal Health Care go through a complete background check through the state and federal level. All employees are fully insured and bonded through the agency. Our nurses and home health aides are skills' tested and put through a thorough interview process in which they meet with three members of our office staff. Each candidate is then carefully reviewed with professional and personal references and approved by the owner.
