About Partners In Care
Partners in Care is a Nursing Concierge Service run by an experienced Emergency Department/ Critical Care RN with over 15 years of nursing care experience. My service is designed to add assistance to those who are independent in their living, however prefer a professional RN to assist in managing their home and health care needs. I provide weekly or bi-weekly client check ins- I do what you need me to do!
-Home/Hospital/Hotel Care- (pre or post surgery, including elective surgery care
-MedicationManagement-picking up/ organizing medications for the week/month,
updating medication lists; making calls to MD office for refills if needed
- Professional RN assessments observe/ watch for any new medical
needs/conditions/decline in pt health in between visits
-Blood pressure checks/ Blood Sugar monitoring, wound care, Infusions, ortho
care among others **with MD orders when required
- Weekly communications to family who may be out of town- I will be your eyes
and ears!
- Attend important appointments to make sure right questions are asked,
and information is understood by client. Information can then be relayed
correctly to family who may live out of the area or unable to attend the appointment
- Transitional Care assistance- any assistance transitioning from home to a greater
level of care I can assist with; gathering resources, making calls, taking tours, I
am able to manage your entire move process.
- Arrange Meal Delivery Services and Housekeeping services
- Elective surgery that you'd prefer not to involve family/friends? I provide hotel
care post-op including medication pick up and administration, transportation to
doctors appointments, wound care/ dressing changes.

Currently employed with Dignity Heath for 6 years/ 8 months

$50/ hour, two hour minimum per visit.
