Hello, my name is Margaret. For over 16 years, I have served aging and frail adults in a progressive capacity. It is my passion to assist clients and their caregivers with navigating goals-of-care decisions to ensure their needs are met while personal beliefs and values are honored. OASIS Transitions is a multi-disciplinary service with the core mission of enhancing the quality of life for adults with life-limiting conditions. We offer education opportunities to health care professionals; referrals for local senior care services; caregiver respite and end-of-life care support.
