Nashua Adult Day Health provides personalized services to seniors and adult with disabilities. This brand new facility offers transportation to and from the program, nursing supervision, medication management, case management, daily breakfast and delicious catered lunches, exercise classes, field trips, educational classes, physical/speech/occupational therapy, group activities and more. All of these service are at NO COST to the member as long as they get a physician referral and are on Medicaid, Choices ForIndependence, Well Sense, New Hampshire Healthy Families or the VA. For those without insurance, the private pay rate is only $75 per day. …
