My Visiting Nurse is a home health care agency that is state accredited for skilled and non-skilled private duty care, RN, LPN, CNA, HHA. Programs to manage activities of daily living, mental well being and counseling, fall prevention, new baby programs, sick child care, chronic disease management, IV therapies, transportation, family respite and more.

Program goals include preventing re-hospitalization, regaining independence, improving patient participation in daily activities, improving physician-patient communication, and providing loving & compassionate care for patients.

Our mission is to improve the quality of life for our patients through clinical excellence, extraordinary service and compassionate care.

Accepted forms of payment include private pay, long term insurance, Medicaid waiver, commercial insurance, workers compensation and Veterans programs.
