We truly believe that if a patient is given the right medical assistance they will thrive health wise, thereby decreasing ER visits and increasing the quality of their lives. We truly believe that with our services it will allow your clients to stay as independent as possible. We would like to provide the following services to your clients: Accompanying them on their Medical Doctor visits, educating them on the Plan of care created by the Provider, their disease processes and their medications. Also, following them while they are inpatient to make sure that they are receiving the best care, consulting with inpatient staff to confirm that all the needs of the clients are met before being discharged, visiting the client in the home post-discharge to assist with a smooth transition back in to the home.
