Medication Management

*Nurse visit Every week

*Weekly Medication set-up by Nurse. Licensed Nurse Comes to your home and sets up your weekly Medication in pill box

*Your Nurse ensures prescriptions are renewed on time and ready for pick-up at pharmacy.

*Your Nurse Updates Medication list and reviews it with your Doctor as needed.
Weekly Nurse assessment as needed.

*Monitors Blood pressure, Heart rate and respirations, and blood sugar levels.
Reports changes or concerns to family and Doctor.

Medication Management + Home Care

In addition to Medication management we also provide Home Care. A Home Health Aide comes to the home once or twice a week to provide care in various ways such as:

Assist with bathing
Assist with activities of daily living
Light Housekeeping
Meal Prep
Medication reminders
