Our movement disorder specialist and geriatric care manager will design a care program to help your loved one reach & maintain the highest optimal & functional level possible in the home. #3.CHRONIC SYMPTOM MANAGEMENT These services are designed to reduce frequent trips to the hospital & rehab facilities. The care programs are designed by therapists, nurses & geriatric care specialists. #4. CARE MANAGEMENT & CARE NAVIGATION: Care managers and care navigators help families who are caring from far away to lower their stress and worry. The goal is to make long distance caregiving easier and more gratifying.. With us, you will only pay for the care you need.
