About Lantern West Health
ABOUT US
Lantern West Health provides committed and compassionate home care services to adults, seniors, individuals with short-term/long-term injuries/illnesses and individuals with intellectual and developmental disabilities (I/DD).
We provide home care to those who wish to remain in their homes, to those in independent living communities, 55 and up communities, and assisted living facilities. We have flexible hourly or live-in care anytime 24 hours a day, 7 days a week.
Our registered nurses supervise andperform training for our caregivers. We also offer continuing education/training for our caregivers.
We are certified by Ohio DODD to provide home personal care (HPC) services in the State of Ohio to individuals with intellectual and developmental disabilities. We are approved for Individual Options Waiver (IO Waiver), SELF Waiver, Level 1 Waiver and DD Waiver Nursing.
We also accept private pay for adults, seniors, individuals with injuries or illnesses on a short-term or long-term basis.
YOUR SAFETY
Your safety is very important to us. We do background checks (BCI) for all our employees before hiring. If they have not lived in Ohio for 5 years, we perform FBI checks. We also check all required registries before hire and enroll employees on a site that continuously monitors and informs us on any violations to the safety checks after being hired.
OUR SERVICES
PERSONAL CARE ATTENDANT (PCA)
PCA services enable you to remain safely and independently at your own home. Tasks provided includes but are not limited to assistance with activities of daily living, medication reminders, shopping, cooking, light house-keeping and taking you to your doctor s appointments.
COMPANION CARE
Our compassionate and caring caregivers will provide companionship, respite care, perform light housekeeping, shopping, cooking, doctor s appointment, bed making, laundry and other activities to fit your needs.
HOMEMAKER PERSONAL CARE
Homemaker Personal Care services for those with Intellectual Developmental Disabilities to help foster and maintain independence in their homes. We follow the Individual Service Plan (ISP) and/or Behavior Service Plan (BSP) to make sure that all the outlined needs are met. Tasks may include but are not limited to grocery shopping, light housekeeping, meal preparations, medication administration, assistance with activities of daily living (ADLs) and transportation.…
