Km Care Management

KM Care Management is a tool to help improve individuals lives and make life easier for them and their families. I have been working with adults with disabilities and seniors for 15 years. First I would evaluate and come up with a care plan based on the individual. Normally I oversee their health care, doctors appointments and medication distribution. Make sure they are receiving the care they need and help with coordinating care. I also assist with day to day activities from getting dressed, exercising, dietary needs or running errands. I have been a case manager for many years but enjoy working directly with the individuals and improving people's lives. I have also been a group home manager and care coordinator.
