Keystone Care is a charitable nonprofit organization that provides a full continuum of home health and hospice services. For more than 20 years, we have been helping improve the quality of life for our patients and their families, regardless of stage of life or state of health. Our services include home care, palliative care, hospice, private duty, and Keystone In Touch Telehealth.

PRIVATE DUTY CARE FOR DAILY SUPPORT
Private Duty assists people who may require help with eating, dressing, bathing, and other housekeeping duties as a result of some physical limitation, and who may or may not have a skilled medical need. It is often requested by patients who prefer to stay in the comfort of their own home and may include assistance with eating, dressing, bathing or light housekeeping duties.

You can begin care without a doctor's referral or insurance authorization; visits vary from two hours to 24-hour coverage daily. Skilled Private Duty care can be provided following a hospital admission, support patients with chronic conditions, or for those who may require more care than the family is able to provide.

HOME CARE
Home care is designed for patients who have been recently hospitalized, have chronic conditions, or for those who need assistance with daily activities or require skilled nursing care.

OUR HOME CARE TEAM

REGISTERED NURSES
Highly skilled registered nurses provide individualized patient care. In accordance with physician's orders, Keystone Care nurses assess the patient's physical condition with special attention to promoting health and healing at home.

PHYSICAL/SPEECH/OCCUPATIONAL THERAPISTS
In coordination with the patient's physician, specialized therapists will assess the needs of home-based patients and develop treatment plans. They will work with the home care team and referring physician to provide therapy services as defined in treatment plans.

SOCIAL WORKERS
Keystone Care's team of social workers provide support and counseling to address the psycho-social needs of the patients, families and caregivers.

CERTIFIED NURSING ASSISTANT, HOME HEALTH AIDE, AND HOMEMAKERS
This team is specially trained to provide personal care to patients such as skin care, bathing, shampooing, shaving, and any personal grooming needs, as well as food preparation or light housekeeping.
