Home-Health Care Partners (HCP) is a private, not-for-profit licensed home care agency providing care in Fulton, Montgomery, Hamilton, Herkimer, Saratoga, Schenectady, Schoharie, Warren, and Washington counties. HCP has an extremely experienced group of RNs, LPNs, certified home health aides, companions, and housekeepers. Our caregivers undergo criminal background checks and are insured. HCP provides case management, safety assessments, meal preparation, housekeeping, personal care, help with errands,activities, and appointments, medication monitoring and assistance, respite, and other nursing/medical care. Care can range from 4 hours to 24 hours a day, including live-in services. Specialty areas include caring for those who need assistance after hospitalization or rehabilitation, and in preparation for diagnostic tests and medical treatments. HCP provides care for complex medical needs, dementia-related difficulties, neurological disorders such as Parkinson s disease and ALS, and chronic, life-limiting and/or terminal conditions that require tailored care in the home setting.
