Hand to Hand is a family owned company established in 2010 with the established mission of providing affordable in home care within the community while creating opportunities for professional growth. Hand-to-Hand is working toward this mission by establishing a sliding scale for patients who have limited finances but want to remain in their home. Owner, Niki Moffett has over 20 yearsof experience as a Certified Nursing Assistant and has done in home care for the vast majority of those years. Co-Owner Raven Patten is a MSW with experience in care coordination. Our intake process involves a medical and psycho-social assessment of the patient with family input included. Once needs are established we then discuss the number of hours needed as well as rate. Sliding Scale eligibility is dependent on patient s income. We guarantee that all new patients will receive services for first week at the rate of $10.00 an hour. We understand that patient care is not a one size fits all approach and we want to develop a unique care plan for patients. Our employees are given a thorough background check as well as a drug screen to protect our clients and families. We service Forsyth, Davidson, Davie, and Stokes counties. We can be contracted to come in home or facility. What We Do.. Bathing Dressing Feeding Light Housekeeping and Meals Laundry Bedbound Management such as turning and changing Companionship Availability 7 days a week, 24 hours a day.
