Great Lakes Adult Day Services, LLC (GLADS, LLC) is a Medicaid approved home health agency that provides home health including medical treatment with the goal of helping you recover, regain your independence, and become as self-sufficient as possible.
Great Lakes Adult Day Services offers a wide range of services that can be provided in your home to help you with recover from an injury, illness, or surgery. Initial visits include an evaluation of medical status, medications, and history. Education regarding medicalcondition and steps to improve understanding and overall health is presented at each visit. All staff work with you to live as independently and actively as possible. Our Home Health team is made up of an inter-disciplinary team of compassionate health care professionals. The services we provide are at the recommendation of your personal physician. We can perform each of the following services based on individual needs: Nursing, Wound Care by a Wound Care Certified RN, Medication administration and Management by a licensed pharmacist, Home Health Aide:light cleaning, shopping, cooking and errands, Fall prevention. In addition, we have a Medication Therapy Management Program. This program helps you and your doctor make sure that your medications are working to improve your health. It also offers monthly refills with delivery. We have a licensed pharmacist on staff that will give you a comprehensive review of all your medications and talk with you about: How well your medications are working, Whether your medications have side effects, If there might be interactions between the drugs you're taking, Whether your costs can be lowered, Other problems you're having.… Read more
