24 Hour Care/7 Days a week upon request without delay.
Personal registered nurse case management specific to each individual.
Medication management by registered nurse as requested by each individual.
Registered nurse to attend each and very doctor's appointment to ensure continuity of care.
Registered nurse to provide reports to family at minimum, weekly and as needed.
Registered nurse to contact doctor if needed for concerns that may arise and request orders to helpavoid future hospital admissions. Registered nurse available as needed 24 hours a day / 7 days a week. Loved ones to be provided with a system, such as Life Alert button, on admission. to be paid for by Graceful Aging, LLC. Personal certified Nurses Assistants are Hand-Picked To Blend With Each Individual's Personality. Personal Certified Nurses Assistant To Provide Transportation As Requested And As Needed. Personal Certified Nurses Assistant, Along With The Individual, To Prepare A Weekly Calendar For Any Upcoming Cultural Events, Community Events, Or Any Other Activities The Individual May Be Interested In.
