Grace Home Health is a Medicare Licensed & Certified home health agency providing a full range of skilled home care services to seniors in the state of Texas. With the aim to keep our clients as independent as possible, we provide the highest quality medical care in the home setting. Our professional staff, including skilled nurses, caregivers, therapists, and others, deliver compassionate care and support when it is most needed.

Skilled Nursing:
-Vital signs
-Head-to-toe assessments
-Wound care
-Diabetic teaching and monitoring
-In-home blood draws and injections
-PT INR checks
-Catheter care
-Tube feeding
-Medication set-up, administration, and management
-Education on disease management and medication compliance.

Home Health Aide:
Home health aide services are covered by Medicare if needed as supportive services for skilled nursing care.
-Bathing

-Light meal preparation
-Light housekeeping.

Physical therapy:
The goal of physical therapy is helping you gain or maintain mobility. Services include: evaluation, gait training, transfer training, home exercise program, therapeutic exercise, and more.

Occupational therapy:
Designed to help you regain your independence by performing daily activities by yourself. You may learn new ways to eat, put on your clothes, or comb your hair.

Speech therapy:
After a stroke or accident, speech therapy can help you regain and strengthen your speech skills through exercise and training.

Medical Social Services:
Social work services are covered by Medicare when given under the direction of a doctor to help you with social and emotional concerns related to your illness. This might include counseling or help finding resources in your community.

Personal Assistance Services:
At Grace Home Health, we know that every individual is unique. This is why we are proud to offer our Personal Assistance Services. Grace PAS will always connect you with an experienced caregiver that is right for you, and our packages are tailor-made for your needs. Through PAS, we are able to provide services that are paid for out-of-pocket, including medication reminders, companionship/sitting, grooming and dressing, safety checks, and much more.
