We are nurse/care managers who are trusted advisers who assist families and senior adults in navigating the complex healthcare system. We offer comprehensive assessment of medical conditions and care needs that will assist families in decision making. Our service model is individualized and allows the senior adult the highest level of independence and autonomy while maintaining safety and dignity. We often are called in to provide guidance when there are complex medical needs and the care path is not clear. Ourpatients usually have one or more chronic conditions which often includes dementia. Our care coordination services allows seniors to remain independent for as long as possible in their own homes and if desired in independent or assisted living. We work closely with adult children and help them to have less stress in caring for their parents/family member. Our advocacy services foster coordination of care with care givers, primary care and specialty physicians, home care services, rehabilitation center therapists and social workers. We also provide guidance related to end of life care decisions including hospice care. We can provide nursing assessments and transitional care post hospital or rehab stay, ongoing monitoring of chronic conditions, medication management, accompany to physicians appointment , education and care plan development for care giving services, assistance with long term care benefits and veterans benefits.
