As much as possible, Faith Community Hospice LLC wishes to enable patients to direct their own hospice care. Honesty, integrity, compassion and respect is our commitment to the individuals we serve.
Home Care:
At least 80% of the overall care of our patients is provided in their home environment. Private residences, long-term care facilities, personal care homes, and assisted living facilities are the most common places of residence. As always the patient/family unit defines "home".
Respite Care:
Caringfor a loved one facing a life threatening illness can be very exhausting. Respite care (no more than 5 days at a time) may be provided only on an occasional basis to relieve the caregiver caring for the patient at home.…
