Our goal is to provide quality personalized care through appropriate coordination of services to meet your specific healthcare needs. Under the direction of your physician, our team of qualified health care professionals will provide on-going case management, if needed, that is appropriate, competent, and consistent.
The Agency's Mission is to provide professional and paraprofessional services to clients in their homes, in order to assist them in achieving their highest level of potential in day-to-day self-care activities. We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment of needs from both the client and professional's point of view. Our professional staff delivers the highest standards of home care nursing, physical therapy, occupational therapy and medical social work. Our para-professional staff of home health aides, personal care attendants and homemakers work tirelessly to assist you in those activities of daily living that are so critical to your independence and ability to stay in your own home for as long as possible.
Our goals and services are based on two fundamental philosophical principles: the belief in the innate worth of the aged and disabled individual; and the belief that each individual, regardless of age, race, color, creed, sex, national origin or handicap(s) is entitled to maximize his potential as a human being and as a member of society. It is our contention that the aging process is a normal state in the development of any individual and that chronic disease and disability are, to some degree, a part of that process.
Emmanuel Home Care Services LLC, is privately owned. The Agency is accredited by the Community Healthcare Accreditation Partner (CHAP) and is certified by CMS. We are committed to ensuring your rights and privileges as a health care patient. Many aspects of our services and procedures may be new to you. We have prepared this booklet to assist you in becoming better acquainted with us, to help you understand the home health process and to explain your rights as a Client.
